Preceded in death by his father, Ralph Hoffman and brother, Thomas.
He is survived by his mother, Joan Hoffman; sister, Kathy (Tim) Smith; nieces, Marinna, Carmela and Adrianna Smith; aunts, uncles, cousins and treasured friends.
Visitation: Monday, Dec. 27 at the 72nd St. Chapel beginning at 5pm with a Wake Service at 7pm.
Services: Tuesday, Dec. 28 at 10:30am at St. Pius X Catholic Church.
Interment Hillcrest Cemetery.
Memorials to the family for future designation.
